Registration for Fall 2009 is now open. 

Registration closes on September 2nd and the first session will begin September 12th.

There are 3 ways to obtain registration forms:

1) Forms may be picked up at Rehab Associates, 6525 US 60 (Inside the Paducah Sportsplex)
2) Email apeal@goperformancefit.com or call 270-744-3701 to request a packet be mailed
3) Download forms from the website www.rehabassociatesky.com.
